The 'flattened' caps are probably ceramic disc capacitors.
Probably for the trem circuit.

Hate to say it but you are going to have to draw a layout diagram.

Follow the schematic.

There is a way but not an easy one.

You would have to refit the CD input jack as a Send jack.
Plus a Return jack will have to be installed with a normally closed contact so the signal will pass without a pedal installed.

That and you will have to cut a copper trace between R21 & R22 (100K pad resistors for the Cd input).

I believe that TR2 is the incorrect transistor.

The schematic calls for a BC212, which is a PNP transistor.

Your notation states that a BC182 is installed in that position.
BC182 is an NPN transistor.

Also, I do not know what is going on there at the LTP (TR4 & TR5) but those two transistors should be the same type & hopefully from the same batch so that there characteristics match.

If the control is marked 'B250K', then it is a linear 250K control.

It was never stated exactly what is wrong with amp.

"Doesn't work" does not lead to the opamp.

Not being able to see the whole schematic I can only venture a guess.

The voltages on A128 are all the same.
That tells me that the device is bad.

To me, that amp appears factory.

Yes, right down to the bad solder joint on the bridge rectifier.:(
