Have you verifyed that the Hsink is indeed *ground isolated* with a Continuity test?

Yes, and you can flip the PCB and verify that the heatsink solder pads don't connect to anything - check for swarf, etc.

No that is *only a visual inspection* :trouble
You need a DMM on the Hsink and the other end to CT of PSU
