I had to reproduce this board because it is not available(couldn't even find a schematic)

Here is a PNG file that can be used as a mask for a positive photo resist board (2.5" x 10" actual size). Also included an inverted picture, as you want the ink side down, contacting the board. So depending on your printer you may need to invert the image to do so. Print to a transparency with the most dense in possible. I had to hand fill the traces with a fine felt tip pen because my laser printer image was not dense enough. The original board is pictured as well as my home made one.
